The PDF icon to the right is provided so you may download, save and print the Tender Point Test Form.   To test your tender points will require the assistance of a friend or family member.  Dr. Smith provides the form so each client may complete and submit an initial and monthly test form to assist her in monitoring progress improvement on their FMS Clinical Nutrition Therapeutic Supplement and Dietary Program.  A Total Score of 11 or more of the 18 Tender Points is the criteria that is used to medically diagnose Fibromyalgia.
