INQUIRY QUESTIONNAIRE (Importance of This Questionnaire)
Reviewing the Patient's Personal Information on the Inquiry Questionnaire, before the Inquiry Telephone Consultation with Dr. Smith, will give her the opportunity to prepare an Initial Nutritional Assessment and Financial Plan so that she may recommend the clinical
nutrition analysis, laboratory testing, and/or other service that best
suits the patient's needs and will save time for both the Inquirer and Dr. Smith at the Inquiry Telephone Consultation. Please note that the Head of Household must also be present at the inquiry Telephone Consultation.
ABOUT THE INQUIRY TELEPHONE CONSULTATION
Because
Dr. Smith wants to answer each Inquirer's questions personally and to their satisfaction, she will either talk to the Inquirer when first called or, if unavailable due to a previous committed, a telephone
consultation will be scheduled as soon as possible.
Please Note: If the Inquirer is not the Head of Household, i.e., the person who has the final authority in all financial decisions, please schedule the Inquiry Telephone Consultation with Dr. Smith when the Head of Household is present. The purpose for this request is provided below.

What is an Acute Program?
It is an immediate, even urgent or emergency, need for Dr. Smith's assistance. People seek our help for a variety of reasons. Some are interested in improving their overall health, to address their multiple vitamin deficiencies, biochemical imbalances and organ/gland dysfunctions, and others contact us, periodically, whenever they need our help, similar to those who go to a medical emergency room when in trouble. Some people prefer natural healing over drug therapy and others have allergies to or have had severe reactions to drugs and, thus, cannot use drugs as a method of treatment. Whatever your intentions or needs, Dr. Smith can help you.
Examples of an acute one-time nutritional support program, may be for a rapid recovery from a cold or the flu, sciatic nerve, sprain or broken bone, post-operative surgery, headache, periodic bouts of diarrhea or constipation, discomfort from acid reflux or indigestion, a sudden state of anxiety, or for any reason, to name a few. TERMS
To make informed deicisons about using a new health care service, it is important to start by understanding its language or terms.
The "Patient" is the person who will be using our services to improve their health. We refer to patients as clients, except on this webpage. Also, refer to "Patient - Client Defined' in the lower-right column.
The "Inquirer" is the person who completes the Inquiry Questionnaire
and then schedules the Inquiry Telephone Consultation with Dr. Smith in
order to obtain information about our services. The Inquirer may also be the patient or the head of household (or both) or neither. The Inquirer may be a family member or friend inquiring for the patient or the head of household.
The "Head of Household" is the person who has the final word on all financial decisions related to paying for the patient's services.
If the patient is a miinor, the Parent or Legal Guardian may be the Inquirer and/or Head of Household.
Attention: Inquirer Are You The Head of Household?
It is important to Dr. Smith that each person inquiring about our services has all of their questions answered to their satisfaction and that the decision, to improve the patient/client's health utilizing our clinical nutrition and naturopathic services, is an informed one.
The most important decision one can make in their life is to invest the necessary and vital time, energy and money in their health. If you are not sure about this, click these links and read our two web pages, titled:
After completing the Inquiry Questionnaire on the patient/client's personal information, you will be ready to call our office to schedule the Inquiry Telephone Consultation with Dr. Smith to get all questions answered to your satisfaction. If the Inquirer is also the Head of Household and Dr. Smith is available at the time of your call, Dr. Smith will glad to provide the Inquiry Telephone Consultation at that time.
If the Inquirer is not the Head of Household, in other words, the person who has the final word on all financial decisions, the Inquiry Telephone Consultation with Dr. Smith will be scheduled when the Head of Household is also present.
Three-way telephone conference calls are a solution to schedule challenges when setting up the Inquiry Telephone Consultation appointment to include the Inquirer (i.e. the Patient or if Patient is a child, the Parent or Legal Guardian), Head of Household and Dr. Smith.
We acknowledge that when the Inquirer is not the Head of Household, the Inquirer is often the person gathering this information to save time and effort for the Head of Household and it may seem a bit inconvenient to have to take the time to coordinate their schedule with the Head of Household and Dr. Smith's schedule to set the Inquiry Telephone Consultation before any information can be obtained about our services.
However, our experience, since 1981, has
proven that this effort actually saves time in making important personal and financial decisions, when all questions are answered in one phone call, with
everyone involved present and fully-informed.
Before we realized the value and time-saving benefits of having the Head of Household present at all Inquiry Telephone Consultations, we would observe that after the Inquirer had made an informed decision that our services were the answer to improving the patient's health, having had their questions answered to their satisfaction, and Dr. Smith had agreed to accept the Patient/Client's case, the Inquirer was rarely able to, then, answer the Head of Household's questions to their satisfaction, due to the complexity of clinical nutrition and naturopathy. This would, then, result in the Head of Household making an "uninformed" decision to not pursue our services.
However, in each case when the Inquirer was also the Head of Household, or the Inquirer had arranged for Dr. Smith to answer the Head of Household's questions, first, the Head of Household made an informed decision to pursue our services and, thus, the patient was now, finally, able to get to the cause of their health challenges and improve their health.
Because it is the patient who always suffered when denied the services that could truly help them, and we want to do all we can to make sure they have this opportunity, it is now our policy that the Inquiry Telephone Consultation with Dr. Smith is scheduled only when the Head of Household is also present.

PATIENT - CLIENT DEFINED
You may notice that we use the term "client(s)" instead of "patient(s)" on each web page, except this one. Why? Because the word "client" represents an inter-dependent relationship
between the patient and Dr. Smith.
What is an inter-dependent relationship?
This means that to improve the patient's health is dependent upon the
success of Dr. Smith's advice and services, plus the patient's ability
to adhere to Dr. Smith's advice and instructions, precisely. Because
it takes both to succeed, to us, the term client best describes this
inter-dependent relationship, than the term patient. Once the Inquiry Process is completed, i.e., completed the Inquiry Questionnaire, the Inquiry Telephone Consultation with Dr. Smith and has paid for
the patient's Initial Assessment, patients are then referred to as our client.
IMPORTANT MESSAGE TO THE INQUIRER FROM DR. SMITH
"I am looking forward to answering all questions to your satisfaction and that of the Head of Household, no matter how tough or impossible they may seem right now.
I am confident that, together, we can formulate a Nutritional and
Financial Plan that best suits the patient's health improvement needs and family budget.
If your intention is to improve over all health, beginning with your most challenging health concerns and because
the lifespan of the red blood cells is three months, I would like for you to commit to, at least, a three-month clinical nutrition
program, in order to:
1. complete the first cycle in the healing process of biochemistry so that it may be sustained and you will will receive value for your three-month investment, even if you choose to discontinue your program in the fourth month, for any reason. Remember, the average time it takes to begin feeling better, no matter what the health challenge, is 3-6 weeks.
2. allow sufficient time for a fair evaluation of the value of our services to you and to prove to you that our clinical nutrition and naturopathic services are safe and effective, and the information in this website is the truth.
3. prove to yourself, from your own experience, that clinical nutrition and naturopathy works!
