Lab Requisition for your Doctor

Preparing For Lab Testing Instructions

     Print the "Preparing For Lab Testing Instructions" and follow the information prior to laboratory testing. Choose one of two methods provided to obtain laboratory tests for your Clinical and Sports Nutrition Analysis:


DirectLab, WSN's Associate Lab
     Our associate lab, DirectLab, has laboratory sites throughout the United States. Click on the DirectLab icon to obtain DirectLab's phone number and to obtain information about laboratory testing. When ready to call DirectLab for the location of the lab nearest you, you will want to provide them with the following information:

1. The date you will be going to the lab site so they can have your order faxed to the laboratory.
2. Our code "DFS" so they will know where to send your laboratory results. DirectLab will also send you a copy of the results.
3. You will also want to inform them that we are ordering the Comprehensive Wellness Profile, unless otherwise instructed by WSN.

     DirectLab has also provided a toll free number to listen to a recording of their services. For Toll Free Recording call, 1-888-286-4718.

Your Doctor
When ready to see your doctor to have your lab tests run, follow these instructions:

4. Click on Laboratory Requisition for your Doctor and type in the information requested.
5. Print a copy for your doctor.
6. Email a copy to WSN by clicking SEND NOW.

     After having your lab tests done, please follow-up with your doctor's office to insure they have made a timely mailing or faxing of your lab results to WSN. This should take no more than seven to ten (7-10) days. When following through, if you learn the doctor's office has not sent the results to WSN, you may want to request a copy and fax it directly to WSN, and then mail the original, to expedite the processing of your clinical and sports nutrition analysis.


     Special Notice: The fee for drawing blood is paid directly to DirectLab, your doctor or the WSN Healthcare Provider who referred you. Laboratory fees are not included in the Clinical and Sports Nutrition Analysis Fee.
